Replacing a Lost or Stolen passport
Police Report
You must obtain a police report from the Shanghai Entry & Exit Bureau before you can apply for a replacement passport. That office is located at:

Although the Shanghai EEB report alone is enough to get your exit visa, in many cases, the EEB will expedite your request for a report if you bring them a report from the local police office where the loss occurred.
Please note: our office can assist only in issuing a replacement passport. Following the issuance of a new passport, the Chinese government requires that a new visa be obtained for the replacement passport. The Shanghai Entry-Exit Bureau (see address above) can issue new visas for replacement passports. Failure to obtain a replacement visa can result in a fine or law punishment when attempting to exit China.

Personal Appearance
All replacements for lost or stolen passports must be applied for in person.
In the case of an applicant under age 16, both parents should also come to the Consulate, or one parent may apply with the notarized written consent and passport copy of the other.
Passport Application (DS-11)
Please submit one copy of Form DS-11, available for download. Though the instructions for world-wide use say only 8.5" x 11" paper is acceptable, we can accept A4 sized copies if you print it out at home. The form is also available in our office.
Affidavit (DS-64)
Please also submit a detailed affidavit of the circumstances surrounding the loss or theft. Here is the form to use, available for download: DS-64. Please don't sign the affidavit-- you will need to sign it at the Consulate when you apply in person for a new passport.

Proof of Your Identity
ID can be proved through a government-issued photo ID document. These include but are not limited to a US drivers license, U.S. state ID, expired passport and/or Military ID. All documents must be originals.
Proof of Your Citizenship
Citizenship is proven through an official birth certificate or naturalization/citizenship certificate, a Consular Report of Birth and/or an expired passport. All documents must be originals. Social Security cards, drivers licenses, Military IDs and/or orders are not proof of U.S. Citizenship.
Two Identical Photos
Unacceptable photos are the number one cause for delays in our processing your passport.
Photos must be 2" X 2" (5 cm X 5 cm) with a white background.
Please be careful with the size-- "passport photos" you get at a local photographer's shop are often too small and are intended for Chinese people to use to obtain a Chinese passport. The photo in your current US passport may be a different size from what is needed for a new passport, as standards have changed from time-to-time.

Fee
The fee to replace a lost or stolen passport is US$100 (RMB 710) for adults and US$85 (RMB 603.5) under those age 16 or younger.
We accept US or HK dollars, or VISA, Mastercard, Discover, Diners Club or American Express card.

How Long Does it Take?
Once we have everything we need, normal processing time is normally about 10 business days. However, please don't make any unchangeable travel plans until you have the new passport in hand. Processing a replacement passport may be longer than normal, if, for example, you have shown a pattern of multiple losses.
Urgent Cases
We work on replacement passports only during regular business hours, except in the case of clear, documented, life-or-death emergencies, such as the death of a close family member in the U.S. Otherwise, no weekend or after-hours assistance is available.
In such unambiguous, extraordinary and fully documented emergencies, emergency passports can be issued the same day. Such passports issued abroad are valid for no more than 12 months (and may be issued with much shorter periods of validity), and must be replaced by a full-validity passport printed in the U.S. You must return the limited validity emergency passport to obtain a full validity (5 or 10 years validity) passport.
