Replacing a Lost or Stolen passport
If your passport was lost or stolen in or around Shanghai, you must obtain a police report from the Shanghai Entry & Exit Bureau (EEB) before you can apply for a replacement passport. You will also need this report to apply for a replacement visa or Chinese exit visa.

Although the EEB report alone is enough to obtain your exit visa, in many cases, the EEB will expedite your request for a report if you submit a report from the local police office where the loss occurred.
Please note: our office can assist only in issuing a replacement passport. Following the issuance of a new passport, the Chinese government requires that a new visa be obtained for the replacement passport. The Shanghai Entry-Exit Bureau (see address above) can issue new visas for replacement passports. Failure to obtain a replacement visa can result in a fine or detention by law enforcement authorities.

All replacements for lost or stolen passports must be applied for in person.
All applicants under the age of 16 must meet the requirements listed in the Law on Passport Applications for Minors. In most instances this means that both parents must sign your child's passport application, even if one parent is not an American. All parents or guardians must present proof of the child's birth or adoption to verify their relationship to the child.
Parents can consent in one of two ways:
Both parents, even if one is not an American, can come to the Consulate with their passports and sign in person. If one parent is not present in China, s/he may give his/her consent through a written affidavit, the DS-3053 form. (The form must be notarized by a notary public in the U.S or at a U.S. embassy or consulate within the previous three months).
Please note that the parent who signs in person will need to still have a copy of the non-present parent's passport with him/her.
Please bring the following to your appointment:
- Passport Application form DS-11
- Statement Regarding a Lost or Stolen Passport form DS-64
- Proof of your Identity - ID can be proved through a government-issued photo ID document. These include but are not limited to a US drivers license, U.S. state ID, expired passport and/or Military ID. All documents must be originals.
- Proof of Your Citizenship such as an official birth certificate or naturalization/citizenship certificate, a Consular Report of Birth and/or an expired passport. All documents must be originals. Social Security cards, driver’s licenses, Military IDs and/or orders are not proof of U.S. Citizenship.
- One Passport Photo (must be 2" X 2" (5 cm X 5 cm) with a white background)
- Appropriate fee
